What We Do

Provide the safety, support, and encouragement that families and teams need in order to build strong and therapeutic outcomes.

Our service model is clinically informed and based on best practices and the tenets of social work and case management.  The program is not “one size fits all,” it’s tailored to each child’s individual needs.  A child does not need to play (or love) sports to be involved in the program; the benefits are directly tied to healthy social and emotional development.

Outreach, Matching & Training

Eligibility is determined for all kids and teams that apply, and is based on a set of physical, social, and emotional factors.

Team Selection
Factors like proximity to campus, physical and sensory needs, and prognosis are considered for each child. Criteria for team selection include size of team, coaching philosophy, team cohesion, and age and maturity of student athletes.

Developing a Personalized Game Plan

Proactive Guidance and Support
Masters level social workers and child life specialists facilitate relationships between kids and their teams, ensuring that they build rapport, connection, and trust.

Personalized Goal Setting
Case Managers work with families to identify personalized goals and develop a plan to achieve them.
Clinical Interventions: Case Managers use a range of therapeutic techniques, including active listening, validation, scripting, reflection, and anticipatory guidance.

Enhanced Support Initiatives
Case Managers provide individualized education and support to teams about topics including self-care and coping with grief and loss.

Achieving Goals

Reaching Goals
Case Managers asses each participant’s progress to ensure goals are achieved and milestones are reached by Graduation.
Achieving Independence: Over time, relationships between families and teams become less reliant on clinical support and intervention and progress towards Graduation.

The formal facilitation provided by Case Managers ends at Graduation, but the relationships can continue on. This is a time to reflect on and celebrate the achievements attained.
